Evaluation of diaphyseal osteotomies for correction of moderate to severe hallux valgus

Abstract

Objective: To compare functional (through the AOFAS scale) and radiological outcomes, during the first postoperative year between the first metatarsal osteotomies (modified chevron Vs scarf) in patients with moderate and severe hallux valgus, in Hospital de San Jose between January 2010 and January 2013. Methods: An observational study type cohort by preoperative radiological measurements at 6 weeks postoperatively and 12 months, as well as preoperative functional measurement 12 months postoperatively through the AOFAS scale was conducted comparing two diaphyseal first metatarsal osteotomies in patients with moderate and severe hallux valgus, conducted randomly by three Sub-specialists surgeons in foot and ankle surgery. Results: A total of 69 feet (57 patients) were included with a mean age of 50.8 years, who underwent equally to one of the two selected surgical techniques, no significant differences were found comparing the radiological measurements and functional scale AOFAS to 6 weeks and 12 months between the two techniques, with higher rates for recurrence modified chevron technique 19.35% compared with that scarf was 5.26%. Conclusions: Compared radiological and functional measurements at 1 year follow-up betwen chevron and scarf osteotomies to correct moderate to severe hallux valgus in the hospital of San Jose show similar results for these two, recurrence rates for technical modified chevron are three times higher than those found in treatment with scarf osteotomy.
