Hallux valgus: treatment with chevron osteotomies

Abstract

Objective: The objective of this study was to evaluate prospectively the results of the treatment of hallux valgus with the use of three different osteotomies, comparing the preoperative and the postoperative clinical evaluation and radiografic measurement, regarding to the degree of correction and AOFAS (American Orthopaedic Foot and Ankle Society) index. Methods: Thirty-seven patients (36 women and one man, from 14 to 74 years), in a total of 64 feet with hallux valgus, were evaluated and treated, between october of 2004 and november of 2006. Based in the intermetatarsal angle (IM), hallux valgus angle (MF), interfalangic angle (IF) and distal metatarsal articular angle (DMAA), we had indicated some techniques of osteotomies of the first metatarsal (chevron for IM?16 and AAMD?12, biplanar chevron for IM?16 and AAMD>12 and modified chevron for IM>16). As complement, we had indicated Akin osteotomy for IF>7. Results: Considering all the used techniques, it had excellent improvement in all the variables after postoperative (pConclusion: We conclude that the chevron osteotomy type fulfills the objectives in the treatment of hallux valgus, with satisfactory results in the three variants researched, and the compression screws allow good consolidation without postoperative loss of the reduction.
